I've put the adoption form up on the web for test purposes.  The form is currently only doing Client side validation.  I am not currently stripping out html characters, etc.  So if you put junk in the form--it will be emailed to you-if the client side validation doesn't catch it. I really want it to work for Mickaboo, so I'd like the tech team to take the first round of testing.  I realize everyone is busy--so if you can just try out the form in one of the ways specified below (As an adopter, fosterer, etc.) and give me feedback I would appreciate it.
 
 
What the form does now:
 
It will hide and show the Foster and Adoption Section based on the checkboxes in the info area.
It will validate the required fields (client side)-marked with the asterisks.
It will email you the form data in a simple html format based on the email you put in the email field.
The submittal takes a second or two--I will optimize and compress the code once we finalize the form layout and destinations for the data.
I have tested it on IE7, Firefox and IE8.  There is a styling issue in the Adoption and Foster Section on IE7 and IE8.  I will fix it once we finalize the design
(I will say I don't know how happy I am with the Spry validation--I usually use another framework.  I used Spry, because it was such a long form and Spry auto inserts with Dreamweaver. )
Also I made a last minute change and deleted the From part of the email-will fix a bit later--you'll see what I mean--Eventually we will put applications@mickaboo.org or something similar

Decisions to be made:
1. Do we want to send the form as a pdf attachment?
2. Do we want to send as a pdf and submit to the db?
3. Do we want to send as pdf and submit to db?
4. Do we want to just send as an HTML email and have a Mickabo0 volunteer print it out to pdf? (I will format much nicer than it is currently)
What I would like to test?
If everyone would try the following:
Take a screenshot of the form before you submit (Alt-print screen on the PC--Mac users you are on your own :)
Submit the data-check your email-verify what you sent is what you got.
 
Any problems : Please send me the browser version, screen shot, and a description of the problem
 
Also please comment as to ease of use-what makes sense what doesn't.
